Della Maddalena opened with the pressure and stance changes he needed, but Makhachev's first level change arrived quickly and put the champion on the mat. Makhachev stayed attached through the scramble, controlled the back and repeatedly threatened the neck. Della Maddalena survived the front-headlock danger, but the early control established the fight's central pattern.

Makhachev returned to the single leg early and used mat returns to keep Della Maddalena from rebuilding his stance. The challenger did make his way up, but every escape led to another clinch or another stretch of top pressure. Della Maddalena's ability to scramble limited the damage, while Makhachev banked another control-heavy round.

Della Maddalena tried to create a more competitive striking rhythm, yet Makhachev continued to mix kicks with well-timed entries. The champion's pressure forced the challenger backward and made every fence exchange a wrestling problem. Makhachev's positional control and short ground strikes kept the round moving in the same direction.

The fourth round became another test of Della Maddalena's defensive grappling. Makhachev closed distance, secured control and repeatedly made the challenger work back to his feet before settling him again. Makhachev finished the title fight in command, keeping Della Maddalena grounded and denying the late striking rally the challenger needed.
